Medical forms

Medical forms are required for different activities. please choose the one most appropriate or what has been requested.

Part A/B – for activities under 72 hours

Meetings, hikes, weekend trips, service projects, etc.
Part A/B must be renewed annually and a hard copy is with the Troop during every activity.

Part C – for activities over 72 hours

Primarily summer camp for most scouts.
Part C must be renewed annually and requires a Dr signature. A copy is not held by the Troop, you need to keep this and only provide it when requested.

(Part C is the last page)

Troop communications

The Troop uses Discord to communicate with Scouts and Scouters alike.

Download the app to your and/or your scouts phones or provide access via a computer.

All youth forums have 2 adult registered leaders in them; this is to ensure youth protection policies are met, however this is not for all parents to join, just the 2 adult Scouters. It’s where scouts do their planning and discussion for activities and events and not having their parents watching/commenting provides a level of responsibility and autonomy on the Scouts.

There are adult only forums, so please have your own log in as well.
